Can someone use a birthday promo and loyalty points on the same booking?

Only if you have configured them to combine. If not, the system blocks loyalty alongside a promo code. Gift cards remain usable either way.

Why did points not move after a visit with no charge?

Points are calculated from the amount actually paid. A completed visit with nothing charged earns nothing, which keeps the ledger honest and tied to real revenue.

Are loyalty and gift cards the same feature on the subscription?

They are separate. You can run a points programme without gift cards, or offer gift cards without loyalty, depending on your plan. Most clinics find the most value running both together.
